
Rachel Meadows is a certified Family Nurse Practitioner who believes in providing high quality, affordable care to the Roanoke Valley. She also wants to ensure that you feel valued, heard, and never rushed through an appointment. She utilizes an integrative approach to care, by blending holistic and traditional medicine.
Rachel is from a small town in Giles County, Va. She grew up watching her father provide care to their small rural town at his private medical practice. In his footsteps, having her own practice was something she always dreamed of.
She received her undergraduate degree in nursing from Radford University and a Master’s degree in nursing from Old Dominion University. She has over 17 years of experience and has worked in a variety of healthcare settings including hospital pediatrics, urgent care, emergency care, family medicine, and health/wellness coaching. In July 2021 she and her family finally made the decision that it was time to do something different. Rachel opened her direct primary care practice in September 2021, where she provides an integrative and modern approach to care.
During her career she has helped patients reach their wellness goals, bridging the gap in care within the medical community. She is passionate about building and maintaining relationships and promoting wellness with her patients.
When Rachel is not caring for her patients, she is investing her time in her husband, Scott and their daughters, Emma and Abigail. She also loves volunteering within her church community at New Hope, hosting dinners with friends, hiking, gardening, and the mountains.

Madi has been dedicated to the field of medicine since 2019. Her medical journey began at the age of 20 when she completed a Certified Nursing Assistant (CNA) program and worked as a Clinical Technician in the Emergency Department. During her time in the ER, she discovered a deep love and admiration for the nursing profession, which inspired her to pursue a career in nursing.
Madi graduated from Virginia Tech with a Bachelor of Science degree in May 2017 and went on to complete the Accelerated Bachelor’s in Nursing program at Jefferson College in May 2019. After becoming a registered nurse, she worked in the Emergency Room for several years and gained experience in Labor and Delivery. Her passion for personalized patient care grew while training under Rachel Meadows during her graduate studies and knew this was her calling.
In 2023, Madi earned her Master of Science in Nursing as a Family Nurse Practitioner from Radford University. She is passionate about providing individualized care to her patients and is committed to making a meaningful impact on their health and well-being.
Outside of her professional life, Madi has been happily married to her husband, Tyler, for five years. Together, they enjoy spending time with their two-year-old son, Tatum. Madi is an avid sports fan and especially loves cheering for her alma mater, Virginia Tech, as well as the Atlanta Braves.
